She is one of the "Sisters" who first appears in Volume 20 of the original novel version of A Certain Magical Index and Episode 19 of the anime's third season. These are the new sisters created in the third production plan (third season). While their sample numbers have not been revealed, they are said to be the youngest of the Misaka series. Like the other sisters, they share part of the Misaka Network. Their appearance is similar to previous Misakas, but they are about 2-3 cm taller and appear to be around the age of a high school student. Like their mother, they have larger breasts than Mikoto, and she claims they are using "different growth promoters." She generally has a malicious personality, preferring to speak in a vulgar and sarcastic manner. Her first-person pronoun is "Misaka." Her twisted personality can be seen in her remarks in the story, such as, "Misaka, your selfish plot developments are making me horny in so many ways☆." While the details will be explained later, she eventually displays a yandere-like side, becoming lovey-dovey towards Accelerator out of jealousy. She also has a weakness for cute things, just like her clone's original, Misaka Mikoto, and her other sisters.

Like her cloned counterpart, Misaka Mikoto, she possesses the ability to generate electricity (Electromaster), a level 4 power level. Her maximum output is said to be 200 million volts. The other sisters' Honesty is only level 2, so it can be said that her abilities are higher than the others. While the original Mikoto uses coins as bullets, she enhances her lethality by firing a railgun using iron nails. As an application of this, she also has the tactic of exploding the air with a huge amount of high voltage current and using that momentum to fly.

Unlike her sisters, she appears more mature than Misaka Mikoto (around high school age). When she first appears, she wears a tight-fitting white combat uniform and goggles that cover her entire face. Unlike Misaka and her sisters, she doesn't speak in the characteristic "and Misaka..." style, but rather in a more extreme tone. Furthermore, her sharp gaze and bags under her eyes are distinctive features of the Misaka Worst, compared to the other sisters.
An individual created solely to kill Accelerator
The Misaka Worst was created with the sole purpose of killing Accelerator, and even if it had been defeated, no other use for it was envisioned. It is believed to have been an individual scheduled to die before the network was officially established in the third production plan. While she is far inferior to Accelerator in terms of skill alone, Accelerator, who has decided not to kill his sisters, does not want to fight her. In addition, she has a "Selector" implanted in her body, which acts as a self-destruct device, neutralizing her without killing her. During a battle with Accelerator, the Special Individual (Misaka Worst) attempted suicide, but was later saved by Accelerator's powers, which also removed the selector and other components from its body.
